Laparoscopic Liver Resection on a 75 Year Old High Risk Patient with H/O Low Ejection Fraction and Post Infarction Changes At RG Hospitals, Goa.

Presenting/Chief Complaints:
75-year-old gentleman, with a case of renal cell carcinoma who had undergone a radical nephrectomy 20 years ago followed by a multi-visceral resection for recurrence 2 years ago. He now developed a recurrence in his liver, which was confirmed by biopsy. It is notable that the patient also had ischemic heart disease, had experienced a myocardial infarction, and had undergone triple vessel angioplasty, resulting in a low ejection fraction and post-infarction changes, making the surgery very high risk.
Diagnosis:
Single metastatic lesion in segment 6 of the liver.
Treatment/Procedure/Surgery:
Laparoscopic Liver Resection under General Anesthesia.
Operative Findings:
The surgery was completed laparoscopically with minimal blood loss, and the patient had an effortless recovery.
Course of Hospitalization and Condition of Patient at the Time of Discharge:
The miracle of minimally invasive onco surgery allowed the patient to be discharged within 2 days.
Highlights:
The final histopathology confirmed that the lesion was completely excised.
This was among the first few liver resections that were done entirely by a keyhole approach in Goa, and we hope that RG Hospitals in Goa will continue to push boundaries in the future.
